The Future of Implementation Science: Pace, Equity, and AI Are Changing the Game
If you work in public health, you’ve probably heard the line: “It takes 17 years for evidence to reach practice.”
It’s become almost a meme: quoted in talks, grant proposals, and policy briefs. But right now, the field of implementation science is quietly asking a much harder question:
How long should implementation take—and what happens when we speed it up or slow it down?
That question sits at the heart of a major new cross-journal Collection, “Advancing the Science and Metrics on the Pace of Implementation,” led by Enola Proctor, Alex Ramsey, and Gila Neta and hosted by Implementation Science and Implementation Science Communications (submissions open through December 31, 2025).
Alongside this call, a wave of recent articles in these journals shows where the field is heading: toward a future where time, equity, and technology are treated as core dimensions of implementation—not afterthoughts.
From the “17-Year Gap” to Measuring Pace on Purpose
The new Collection is unusually explicit: we don’t actually know how long implementation takes. “Speed” is rarely measured, reported, or theorized. To change that, the editors are asking for studies that:

Scan recent issues of Implementation Science, and you see a pattern: the field is less satisfied with “we used a multi-component strategy, and it sort of worked.” You get:
- A scoping review and SELECT-IT meta-framework to help teams choose theories, models, and frameworks more deliberately.
- Debate pieces on mechanisms of change that ask how strategies like facilitation or audit-and-feedback actually move behavior.
- Methodological guidance on how to assess the impact of adaptations—instead of treating adaptation as messy noise.

For public health practitioners, that means more usable answers to questions like, “If I only have resources for light-touch facilitation, how long will it take to see any change?”
Context and Equity Move from Sidebars to Center Stage
Another visible shift is a much deeper obsession with context and generalizability. Collections on “generalizing and context in implementation research” and tools such as IFASIS and IMPRESS-C are being developed to measure context and sustainability determinants in a structured manner. At the same time, there’s a growing body of work focused on:
- Implementation and sustainment of health interventions in African settings and other low- and middle-income countries.
- Humanitarian assistance, where implementation science is proposed to optimize care in settings of displacement, conflict, and disaster—spaces where speed, context, and equity collide in real time.
- Equity-driven implementation in areas like school meals, LGBTQ+ school health, Native American family wellness, and rural mental health.
Put simply, “does it generalize?” is being treated as an empirical question, not just a hand-wavy limitation. And equity isn’t just a rhetorical flourish; it’s baked into the design, measurement, and interpretation of implementation studies.
For public health, that means more evidence you can actually argue with: Why did this work in a large academic system but stall in a safety-net clinic? What about rural? What about multilingual, multi-ethnic communities?
De-implementation, Policy, and the Long Tail of Change
Another visible future direction: Implementation science is becoming the science of what to stop doing, not just what to start. Recent work includes:
- De-implementation trials and reviews on low-value antibiotics, unnecessary nursing care, and over-ordering lab tests.
- Sustainability studies track which interventions stick after the grant ends, and why.
- Policy implementation research on things like tax earmarks for behavioral health, aging-in-place reforms, and mental health strategies—asking what it takes to move from evidence → recommendation → on-the-ground change.
The pace Collection explicitly invites studies on time to de-implement harmful practices and the timeliness of policy implementation, not just the adoption of shiny new programs.
For health departments and community partners, this supports the kinds of questions you live with every day: How fast can we safely unwind a low-value practice? What gets in the way when we try to implement a new law or guideline?
AI, Digital Health, and the Data Layer of Implementation

Finally, there’s a strand of work where implementation science turns the mirror on itself:
- Costing studies that estimate what it actually costs to deliver and participate in strategies like facilitation, coaching, or collaboratives. =
- Evaluations of national training programs and capacity-building initiatives.
- Editorials on grant terminations and portfolio analyses of funders’ investments in policy implementation science. \
That meta-work matters for public health because it informs where limited dollars and people power should go. If we can say, with some confidence, “this combination of strategies is too expensive for the marginal gain” or “this kind of capacity-building changes practice five years later,” then we can stop reinventing the wheel in every jurisdiction.
What This Means for Public Health Practice
Taken together, the pace Collection and the recent wave of Implementation Science / IS Communications articles are signaling a new phase for the field:
- Time becomes a design variable—we’re not just trying to close the 17-year gap, we’re asking what tempo is appropriate for different crises, communities, and interventions.
- Equity and context are foregrounded, with more work in LMIC, humanitarian, and marginalized settings, and better tools for measuring context and sustainment.
- Mechanisms and strategies are more precise, letting practitioners choose more targeted, right-sized approaches.
- Digital and AI solutions are treated as implementation challenges, not magic bullets.
- Costs, capacity, and policy realities are studied, not ignored.
